2008 Hall of Fame Reservation Form

The United States Martial Artist Association Hall of Fame Banquet will be held July 19, 2008, at the Holiday Inn  Ft. Detrick, Banquet Center in Frederick, Maryland. We are anticipating an overwhelming response, so please make your reservation as early as possible. Please complete this reservation form and return at once along with your check or money order in the amount of $85.00 per person (Guest and/or Inductee) (sorry not refundable if you decide later not to attend for any reason). Make your payment payable to the SGM William A. Rankin, or pay with paypal located in our catalogue. Your reservation allows you to attend all of the seminars/clinics that will be conducted throughout the day.
